Degenerative Myelopathy in Dogs

Mobility problems are among the most common conditions in the aging dog. There are several causes of this, one of which is called degenerative myelopathy (also referred to as DM). As the name suggests there is a deterioration of a crucial component of mobility. The word myelopathy refers to degeneration of the spinal cord, which is basically the highway allowing the brain to connect and communicate with the rest of the body.

 In degenerative myelopathy, the spinal cord of the lower back (lumbar area) loses its signal conducting covering called myelin. When this happens, the brain cannot send messages along the spinal cord to the nerves, and the nerves cannot stimulate the muscles of the hind leg to move. As a result, the muscles of the hind legs weaken, lose the mass, and the dog loses control of its hind legs. Some breeds are more affected with this condition, particularly German Shepherds and Boxers, but it can occur in other breeds and in mixed breeds as well. The age of onset varies, but most dogs are in their senior years, but it can be found in middle aged dogs as well.


Signs of Degenerative Myelopathy

Many of the signs of DM are like arthritis and hip dysplasia

Changes in gait -swaying, wobbling, tripping, falling, dragging

Scuffing the top of the hind paws causing stubbed nails or bleeding nails

Crossing hindlegs

Atrophy of hind leg muscles

Inability to support weight in the hind legs

Fecal and urinary incontinence in the advanced stages


Diagnosis of Degenerative Myelopathy

Clinical symptoms and physical examination

Neurological examination

Radiographs and blood testing

DNA test to detect a gene mutation leading to this disease

Advanced diagnostics such as MRI and CT scan


Treatment of DM

Routine exercise to maintain muscle strength in the hind legs.

Physical therapy

Boot covers and paw grips to prevent injury and help with traction

Canine carts and support devices to help the pet to stand and move around

Maintaining proper hygiene

Several supplements have been used but their benefits have not proven consistent

Prognosis

DM is a progressive disease over months and many pet reach a disabled state within a year of diagnosis; at which time they are euthanized.
